By agreeing to the following terms and conditions, I acknowledge that I am requesting access to portions of my personal health information and the ability to communicate with my Wheaton Franciscan Healthcare (hereinafter referred to as Wheaton) team concerning my health information via the Internet using an electronic application called MyChart. My continued use of this site indicates my agreement to abide by these terms and conditions.

I understand that MyChart should never be used for urgent matters.
